All documents must be uploaded or else the request will not be processed.

Please have ALL the following documents ready:

1. Birth Certificate
2. Proof of Address
3. Immunization Record
4. Attendance Report 
5. Discipline Report
6. H.S. Transcript / Report Card
7. 504 Plan (if applicable)
8. IEP (if applicable)
