1095-C Form Information

Form 1095-C for the 2024 Benefit Plan Year was mailed to employees on 2/24/2025. 

• Form 1095-C for the 2024 Benefit Plan Year was mailed to employees on 2/24/2025. The 1095-C Form provides information about the District offered health insurance coverage and whether you were enrolled in District health coverage during the 2024 plan year.  While the information on the form may assist in preparing your individual income tax return, it is not required to file a tax return.  If you have not received your 1095-C form in the mail, you can contact the Benefits Department at 209-933-7026 or email: benefits@stocktonusd.net to request a copy.
• Form 1095-B for the 2024 Benefit Plan Year will be sent to employees directly by the health insurance carrier (ex. Kaiser, Anthem, Blue Shield, UnitedHealthcare, etc.). The 1095-B form provides information on which months during 2024, both you and any dependents were enrolled in health coverage. Please contact your medical insurance carrier directly (phone number on the back of your medical ID card), if you have not received this form by 2/1/2025.
