Site information
- If a student is injured, complete the online form within 1-2 business days for minor injury and immediately after the student is treated for major injuries - https://forms.stocktonusd.net/Forms/sar
- This form is routed to Risk Management.
- If a parent requests a copy of the accident/injury report, direct them to Risk Management.
Parent Information
- If your child has been in an accident or injured at school, and you are requesting the record of the incident, please complete the service request form and submit it to the SUSD Risk Management at 55 S. Lincoln St, Stockton, CA 95203.
- Service requests will be processed within 5-7 business days - Service Request Form

Student Athletes Accident/Injury
Site/Coach Information
- If an Athlete is injured the coach is responsible for completing this Athletic Accident/Injury Report Form.
- Send completed form to Risk Management within 24–48 hours from the date of incident to riskmgt@stocktonusd.net (form needs to be complete, signed, and dated before sending to Risk Management)
- Give a copy of the report to student/parent.
Parent Information
- If your child athlete has had an accident/injury while playing a sport please request information from your child’s coach.
- If you would like to file a claim please submit the claim form provided in the Student Accident and Sickness Insurance Claim form that is mailed to you once Risk Management received the injury report from the students' coach.
