School Staff
- Staff must obtain an updated TB assessment every 4 years
- School Staff may obtain a TB Risk Assessment by
- Calling Health Services at 209-933-7060 for an appointment (in person or virtual via computer with a camera)
- Via their Primary Care Provider
- Via their site school nurse (if available)
If a TB Risk Assessment requires a follow-up TB skin test and/or Chest X-ray (determined by a Credentialed School Nurse)
- Employees are referred to the Primary Care Provider for certification.
- Completed forms are sent to Human Resources via email to TBHelp@stocktonusd.org
Parent Volunteers
