This consent form will ensure that your child will receive the services he/she/they might need from the Franklin High School Wellness Center. Please see the services we offer listed below.  Services are free to Franklin High School students. Please return the completed form to Wellness Center. The consent form will be in effect for the duration of a student's enrollment. A consent form may be revoked at any time if desired. 

Este formulario de consentimiento es para asegurarár de que su hijo recibirá los servicios que él / ella pueda necesitar De el Centro de Bienestar en la Escuela Preparatoria Franklin. Consulte los servicios que ofrecemos al final de este formulario. Los servicios son gratuitos para los estudiantes. Por favor devuelva el formulario  al Centro de Bienestar.  El formulario de consentimiento estará vigente durante la duración de la inscripción del estudiante. Un formulario de consentimiento puede ser revocado en cualquier momento si así lo desea.
